Yes. If you have a chronic medical condition that has been present (or is likely to be present) for at least 6 months, your GP can prepare a GP Management Plan (GPMP) and Team Care Arrangement (TCA). This entitles you to Medicare rebates for up to 5 individual allied health consultations per calendar year.
Under the Australian Government Hearing Services Program (HSP), eligible Pensioner Concession Card holders, DVA Veteran Gold/White cardholders, and certain other beneficiaries receive fully subsidized hearing assessments, fully funded digital hearing aids, and comprehensive ongoing care. In addition, free 15-minute adult hearing checks are regularly available to all community members.
No referral is required if you are attending as a private patient. You can simply book an appointment and claim your rebate on-the-spot through private health insurance (HICAPS). However, if you wish to claim Medicare rebates under an EPC/CDM plan, or are funded via DVA or WorkCover, a formal GP referral is required.
You can book by calling Elmwood Medical Centre reception directly on (02) 6056 2011 or by booking online through HotDoc. Our reception team will gladly assist you in coordinating your GP Care Plan and scheduling appointments with the appropriate allied health practitioner.
