Privacy & Confidentiality

We maintain full confidentiality of our patient’s medical records and as such it is the policy of the clinic not to release any patient information or clinical results to patients over the phone. It is important that the doctor discuss the results of any investigations with the patient on a face to face basis, so that any questions can be addressed properly.

Staff at the Elmwood Medical Centre understand that information pertaining to patient health is highly sensitive, and as such our policy and procedures ensure that this information is protected at all times.
All patient information is stored electronically and is password protected. All staff at the Elmwood Medical Centre are required to sign and comply with our confidentiality agreement.

To ensure patients are provided with the best healthcare possible, the practice will collect the following information:
Patient Name
Date of Birth
Phone numbers
Medicare Number
Health Care Card/Pension Card Number
Current drugs/medications
Social and Family history
Current and previous medical history
Further information relevant to providing healthcare as deemed relevant by the clinician.
